For approved volunteers that purchase items for DARA and or Hamvention, here is a PDF Copy of the standard voucher form.  When using this form, you must have the original copy of the sales invoice/receipt and have voucher signed by the approved individual. It must be countersigned by the current DARA President for DARA expenses or Dayton Hamvention General Chair for Hamvention expenses.
